How to register: send the duly completed and signed form along with a copy of the valid Hong Kong identity card or Certificate of Exemption of the mentally unfit person by post to the "Consumption Voucher Scheme Secretariat at GPO P.O. Box 180005, Hong Kong". It is not necessary to affix stamps on the envelope if posted in Hong Kong.
